This consent explains what to expect from telehealth visits delivered by Altro Health and its independent, licensed clinicians through your Basecare membership.

Purpose of This Consent

This Telehealth Consent explains what to expect from telehealth services accessed through your Basecare membership and delivered by Altro Health and its network of independent, licensed clinicians. By requesting or using a telehealth visit, you agree to the terms below, in addition to our general Terms of Service.

What Telehealth Is

Telehealth means receiving care remotely — by secure video, phone, or asynchronous messaging through the Altro Health portal — rather than in person. Your clinician will review the health information you provide and, using their independent clinical judgment, determine what evaluation, treatment, or referral, if any, is appropriate.

Benefits and Limitations

Telehealth is convenient and can reduce the time and cost of getting care compared to an in-person visit. It also has real limits: your clinician cannot perform a hands-on physical exam remotely, and some conditions require in-person care, additional testing, or a referral. Your clinician will tell you if that applies to you.

Not for Emergencies

Telehealth through Basecare and Altro Health is not appropriate for medical emergencies. If you are experiencing a medical or psychiatric emergency, call 911 or go to the nearest emergency room immediately.

Your Clinician

Clinicians delivering telehealth services are independently licensed in the state where you're located and are affiliated with Altro Health, not employed by Basecare. They make all clinical decisions, including whether to prescribe medication, independently of Basecare. Basecare does not review, direct, or influence those decisions.

Privacy of Your Information

Information shared during a telehealth visit is handled under Altro Health's Notice of Privacy Practices and applicable law. See our HIPAA Notice for how information is divided between Basecare's membership platform and Altro Health's clinical systems.

Technology & Your Responsibilities

  • Use a private, secure location and a stable connection for telehealth visits.
  • Keep your login credentials confidential.
  • Provide accurate, complete health information — inaccurate information can affect your care.
  • Technical issues, such as dropped connections or device problems, can occasionally interrupt or delay a visit.

Your Right to Withdraw Consent

You can decline or withdraw from a telehealth visit at any time, before or during the visit, without it affecting your Basecare membership. Declining telehealth may mean seeking in-person care instead, which Basecare and Altro Health do not provide directly.

State-Specific Requirements

Some states require additional disclosures or a separate verbal or written consent captured by your clinician before your first visit. This page is a general summary and doesn't replace any state-specific consent your clinician collects directly.
